Skilled Physical Therapy Evaluation

**PT Evaluations do NOT require a referral prior to the evaluation as of 11/1/19.

We provide skilled physical therapy evaluations and treatment sessions to children in the comfort of their home. We come to you!

During a Pediatric Physical Therapy Evaluation, the therapist will:

design a specific evaluation tailored to the needs of your child
obtain relevant background information, patient forms, referrals and past medical history in order to paint an accurate and specific picture of your child’s developmental health.
perform objective assessment and application of standardized testing tools including the AIMS and PDMS-2
develop a treatment plan for the child including specific goals for therapy as well as a tailored home exercise program to promote optimal results

This service typically averages 60 minutes

Wellness:

**Wellness visits do not require a referral from a doctor

We provide health and wellness consultation to parents and their children for typically developing children in the comfort of the home. We come to you!

During a Wellness Visit, the therapist will:

obtain relevant background information, patient forms and past medical history in order to paint an accurate and specific picture of your child’s developmental health
assess the child using age-appropriate tests to examine reflexes, positioning skills, strength, coordination, and function
answer any questions or concerns from the parent about the child's developmental progress
teach typical age-level gross motor (movement) skills, what to expect during the next few months of development, and how to continue to boost your child’s growth

This service typically averages 45-60 minutes
